Before you act: This guide is informational. Official immigration rules, insurer terms and coverage eligibility may change. Verify current details with the relevant Czech authority and the insurer.

GUIDE 1 / 5

First: check each family member's residence status

  • There is no single insurance rule for every family. Your spouse's, partner's and child's requirements can differ depending on employment, EU rights, residence permits and the type of visa.
  • A parent employed in Czechia may qualify for public health insurance, but that does not automatically mean a non-working spouse has identical entitlement. Verify each person separately.
  • Before buying private insurance, confirm if each person is already entitled or required to join public health insurance.

GUIDE 2 / 5

Children under 18 with a long-term residence permit

  • Since 1 January 2024, foreign children under 18 holding a valid Czech long-term residence permit generally participate in the Czech public health insurance system. For these children, enrollment is mandatory rather than an optional substitute with commercial insurance.
  • This rule does not automatically extend to a child who holds only a long-term visa or a temporary residence permit as a family member of an EU citizen. Check the child's exact status before purchasing a policy.
  • Public insurance generally starts when the child collects the first long-term residence card. Contact a Czech public health insurer promptly to confirm the registration procedure and applicable deadlines. The child's legal representative, guardian or trustee is responsible for the premiums.

GUIDE 3 / 5

Long-term visa and family-reunification applications

  • For an initial long-term visa application, a child or dependent adult may need suitable commercial medical insurance if they do not already qualify for public cover or another accepted arrangement.
  • Check the embassy or Ministry of the Interior requirements for the specific application. Different rules can apply to long-term visas, residence permits and family members of EU citizens.
  • If commercial comprehensive insurance is required, compare insured persons, period, exclusions, provider network and the insurer's official documents before proceeding.

GUIDE 4 / 5

Pregnancy, newborns and transition periods

  • Pregnancy and childbirth benefits, waiting periods and pre-existing-condition exclusions require particular attention. Confirm actual policy terms in writing before relying on a product.
  • A child born in Czechia can have different public-insurance rights depending on the parents' residence status. Some cases require an application for the child's residence permit within 60 days of birth and public-insurance registration within eight days.
  • Do not leave gaps between commercial cover and public-insurance eligibility; ask the public insurer and residence authority to confirm the dates relevant to your family.

GUIDE 5 / 5

What to prepare before applying

  • Gather passports, dates of birth, expected entry dates, visa/residence categories, employment status and planned length of stay for each person.
  • If someone is eligible for public insurance, contact a Czech public health insurer for enrollment steps; if commercial coverage is needed, calculate it individually and continue through the official Slavia application.
  • Keep the issued contract, insurance certificate and proof of payment, and verify which documents your embassy requests.
